This is horrible to hear. I feel for the families and I pray their strength even now. I previously worked in the dental field and the doctors in a family practice would not normally sedate a young child and they would refer them to a Pedio dentist if it is too much of a challenge with that child.
The second dentist should be brought up on charges, because that to me was gross negligence. I do know that sometimes the parent's are asked to wait in the waiting room. But what about the dental assistant that had to be present in the room. She for one, should have seen the gauze and removed if the doctor had not seen it and the procedure was completed. But we don't really know if this was done prior to the procedure being finished or after.
